COPD Transitional Care Program

The COPD Transitional Care Program allows you to be cared for in a post-acute care setting instead of in the hospital. Our skilled clinical team of Respiratory Therapists, Nurses and Rehabilitation Specialists will provide comprehensive education and rehab to provide you the self-confidence to manage your COPD at home. You will be discharged from our program in 7 days to go home.

HOW WILL THE PROGRAM BENEFIT ME?

By participating in the COPD Transitional Care Program at Islands Skilled Nursing and Rehabilitation you are assigned a personal Pulmonary Disease
Navigator who will oversee your Pulmonary Rehab and education during your 7 days stay. Our patient centric program is designed to help reduce or prevent complications and may even reduce the possibility of emergency treatment or hospitalization.

WHAT YOU NEED TO KNOW

How the program works:
  • You will be discharged from the hospital to continue your care for 7 days at Islands Skilled Nursing and Rehabilitation then you will be discharged home from Islands.
  • You will receive comprehensive knowledge of COPD, education on relaxation breathing and shortness of breathing.
  • Our team of Respiratory Therapist will provide bedside Spirometry Testing to determine your lung function and a Six Minute Walk Test to determine your requirement for oxygen will be performed during your stay.
  • Our Rehab Team will provide you with a Pulmonary Rehab assessment of your strength and functioning and identify your activity goals.

WHAT HAPPENS NEXT

After discharge:
  • You will be enrolled in our COPD Care at Home Program.
  • The COPD Care at Home Program is a Remote Patient Monitoring (RPM) program which allows our skilled clinical team to remotely monitor your condition from the comfort of your own home. Your doctor will review your data and can adjust your treatment plan as needed.
  • You will be partnered with our Hawaii-based patient monitoring specialist – Kaleen.
  • Kaleen will call you from (423) 225-1553.
  • Together you and Kaleen will set up your device and get you on the path to monitoring.
